How write two Input in a pop-up GUI

With the following code I have one variable definde as $x for the ok.  I would like to add User_name for the variable $u, this would be the second.

Not sure what part can be duplicated and how to insert the variable for the OK button click.    So both texted entered would be passed on to the next part of my script.
